Sitagliptin is an antihyperglycemic medication used to help control blood glucose levels in type 2 diabetes. The medication inhibits the secretion of hormones typically released in response to food, which encourages the pancreas to produce more insulin and inhibits the liver from producing glucose.
Sitagliptin is more widely recognized by the brand name Januvia. It's also possible to buy sitagliptin combined with the widely-prescribed metformin under the brand name Janumet.
How It's Used
The most complicated part of treatment with sitagliptin is determining dosage, which is largely dependent on kidney function. Kidney damage is common with type 2 diabetes and function will need to be tested prior to starting treatment; sitagliptin is ultimately filtered through the kidneys and those with damage will not be able to handle higher doses. As a general rule:
- Patients with little to no kidney damage typically take 100 mg per day.
- Those with moderate kidney damage typically take 50 mg per day.
- Those with severe kidney damage typically take 25 mg per day.
It's also not unusual for other anti-diabetic medications to be taken along with sitagliptin; in these cases the other medications may influence dosage, as can other personal variables such as age or weight.
Once dosage is determined treatment is straightforward; doses are taken just once per day, with or without food as desired. Treatment may continue for as long as needed.
Blood glucose levels should be monitored closely at the start of treatment and following any dosage adjustments.
Periodic kidney evaluations are advisable for the duration of treatment. Kidney damage can be very difficult to detect without laboratory testing until severe; severe kidney damage is painful, costly, and can be fatal.
The most common side effect reported is mild nausea. Less common side effects include headaches and symptoms similar to the common cold.
